MD Licensing / Certification Requests

The Vagelos College of Physicians and Surgeons Office of Student Affairs works in concert with the CUIMC Office of Registration Services on providing materials needed by graduates regarding MD degree certification and medical licensing requirements. There is an average of a 7-10 business day turnaround time for all requests.

Licensure Applications

If the document requires completion/signature by a medical school dean, please email the request to the VP&S Office of Student Affairs md_credentialing@cumc.columbia.edu.

If the document requires completion/signature by a registrar, please email the request to cumc-rfs@columbia.edu

Certified Diplomas

Copies of diplomas are not maintained by the college or university.  In order to have a copy of your diploma certified, you must provide a photocopy/picture of your diploma to the Office of Registration Services for certification by the Registrar.  Please provide this document along with your licensing/program forms to the responsible office detailed above.

Official Transcripts

The Official Transcript bears the Columbia University seal, the date of issue, and the signature of the Registrar.  To request, please complete the Transcript Request Form.

Due to the Family Educational Rights and Privacy Act (FERPA) of 1974, the university cannot respond to a transcript request without a legal signature. We are unable to honor transcript requests by e-mail unless the Transcript Request Form has a live signature and has been scanned and emailed.

MPSE/Dean's Letters

Requests for a copy of an MSPE/Dean’s Letter are fulfilled by the Office of Student Affairs.  Copies are never provided to graduates directly and must be sent directly to the final recipient (i.e. residency/fellowship program director, hospital staff office, credentialing agency, licensing board, educational institution, etc.).  Requests should be directed to Norma Pressley, VP&S Office of Student Affairs, mspebio@cumc.columbia.edu
