5-1 Resignation (eTermination)

Policy

A full-time officer of instruction who wishes to resign should give notice in writing as early as possible but no later than three months before the resignation will be effective. In cases of hardship, situations in which a faculty member would otherwise lose an opportunity for professional advancement, or other special circumstances, the Provost may waive the requirement of notice on the recommendation of the dean or vice president. For OOIs in the Faculties of Dental Medicine, Medicine, Nursing, and Public Health, the Provost has delegated this responsibility to the Executive Vice President for Health and Biomedical Sciences.

An officer of research who wishes to resign is expected to give notice in writing as early as possible, but in any event no less than three months before the resignation will be effective.

Requirements

Department submits the following:

  • PAC eTermination, or a PAF if unable to use eTerm (e.g., for an officer with multiple positions)
  • Officer’s letter of resignation, including the effective date and reason
  • Any other supplementary documentation required, such as Labor Accounting Form (LAF) for termination of annual add-comp(s) or vacation tracker or last timesheet

If the officer is on non-immigrant visa (J-1, H-1B, O-1, E-3, TN) submit a Termination Form to ISSO-CUIMC.

Workflow

  • The OFA Representative reviews the request and obtains OFA approval.
  • OFA submits the eTerm or PAF to CUIMC Payroll, if salaried, or directly to HRPC, if zero-salaried.
